Dear Parents and/or Guardians:

It has been a great school year. I hope that everyone has a wonderful relaxing summer.

Here are just a few reminders and announcements.

We will be offering Sports Physicals over the summer on Monday, August 26th from 8am--12pm at the Middle/High School Health Office. All athletes need a current physical in order to participate in sports. If you would like your child to have a physical on August 26th, please complete and sign the included Health History form and bring your child to the school on August 26th at 8am. Students will be taken in the order that they arrive. Please call athletics or the MS/HS health office with any questions.

New York State law requires the following:

Students entering 6th grade

❏ Up to date vaccinations

❏ Students entering 6th grade need a Tdap booster

Students entering 7th grade:

❏ A current physical examination

❏ Students entering 7th grade need a MCV vaccine

It is recommended that all students have up to date physicals on file in the Health office. Please use the summer months as an opportunity to have your child’s physical.

A copy of a physical examination and/or proof of immunization must be provided to the school within 14 days from the start of the 24/25 school year.

The physical examination must be completed by a New York State licensed physician, physician assistant or nurse practitioner and must be dated no more than 12 months prior to the start of the school year. If you are unsure if your student has received the required immunization(s), please contact your primary care physician. If you have already provided the school a copy of the physical exam and/or proof of required immunization(s), then there is no need to submit another copy.

We also would like to remind you that all medications, including rescue and over the counter medications, require a doctor’s order allowing the medication to be carried by students or administered by the school nurse.

We suggest you make copies of the completed forms for your own records before sending them to the school health office. Forms may also be faxed to the number below.
